| Exercise group for frail elderly |
Dear all, I am currently planning an exercise class, mainly aim at fall prevention through balance training and strengthening for the elderly clients (esp. the rather frail ones) living in the community. I'll be using the gym in a community complex with only simple setting e.g. some chairs, some handy equipments which I could bring along with.
May I have somebody share with me your experience on conducting groups like this? I'm interested to know more about group therapy of this kind, please kindly e-mail me:

| Lateral calf pain |
Hi, I have been treating this lady with pain along the fibular mainly around the distal and proximal ends vary rarely in the middle. She is a keen runner and used to run for about an hour 3 times a week with gym visits on top of that including a big weights program and low impact cardio.
The pain started as ITB friction syndrome and as she had extremely pronated feet I
referred her to a podiatrist who gave her some insoles (this lateral knee pain went after some stretching etc). She did her first run with the insoles with no pain when running but that night had really bad pain around the head of her fibular. She's kept resting and running (the pain is now when she runs) and for days after along the line
to the fibular, there doesn't seem to be anything that's working what could it be?

| Gaining finger extension post-stroke |
Hi, I'm treating a woman with a 5 week old right hemi. She has active movement in all UL joints, has full ROM and has no subluxation. She is progressing well but I'm looking for inspiration for what else I can do to stimulate finger extension. She has limited extension in all digits - not enough to be functional. We are using sweeping as sensory stim., active & active - assisted exercises, weight-bearing.
Got any more ideas to stimulate extension as there has been no progress over the last week? There is a slight increase in flexor tone but it is not inhibitory and when it is inhibited she doesn't gain any extension.
All suggestions welcome.

Hello All,
I am searching for a questionnaire for my dissertation. My dissertation topic is "VENTILATORY MUSCLE TRAINING IN QUADRIPLEGIC PTS". I have taken outcome measure as PFT but there is little difference in it although subjective improvement is there.
The patient feels better in speaking, coughing, exercising etc. I want some questionnaire on that can any one help me out in this or how to get that questionnaire on internet because on internet I get their names only not the full questionnaire.

| Differential diagnosis of Lumbar spine symptoms |
If I complain of L5/S1 bilateral symptoms provoked by extension and side flexion of the lumbar spine with the arms at the side but when I have the arm elevated and repeat the same movements and I get no symptoms, better quality of movement- what can I hypothesise as having changed?
Have I improved stability by engaging the posterior sling? Have I reduced the compression on the disc and stretch on the facet joint capsules hence reducing the pain?
How can I promote this movement in our daily lives? Should i stabilise the core or does manual therapy have a role in mobilising the dysfunction? Or indeed both?
I am treating this client with a stability program, education (postural) and mobilising the Lsp PIVM l5/S1 in to rotation grade 3. Her comparable symptoms can also be reproduced with SLR and neck flexion and have started mobilisation of the neural system.
(Neuro tests all completed NAD)

| Spondylolisthesis and disc protrusion |
Hey guys, This is one of the toughest cases
I've come across in my clinicals. plz help !
The patient a 55 year old male has an anterolithesis of L5 over Sacrum ( GRADE I) and Disc protrusion at L3-4 . Patient is also a Bronchitis patient.
The patient presents with buttock pain left sided with radiating pain down the whole lower limb.
I've not assessed him completely for muscle strength, sensation.. etc.. Pain increases on sitting and walking short distances. Also dyspnoea on exertion.
I'm sooooooo confused what to do.. give flexion exercises or extension exercises.. traction or mobilization.. god plz help..

| Taping technique for Metatarsus Adductus |
Hi everyone, I have a big number of patients presenting with metatarsus adductus between 18 months and 3 years that were not treated as infants or have a relapse. I find they do not tolerate stretching or tickles (eversion stimulation) well at this age and wedging the shoes at the hallux does not always work. I have experimented with taping the foot across limiting the metatarsals from adducting. It seems to work for 60-70% of my patients. Has anyone else used this and is there any articles I should be reading?

Exercise prescription for patients with pulmonary fibrosis to improve physical fitness and quality of life. I've read few
articles on effect of exercise on these pts, but they don't say much about what was their actual exercise program, in terms of exercise prescription.
Help me if you've got any information on prescribing exercise for these patients.
